I would normally assume that this cover was meant to be symbolic of the threat of death looming over a life of crime… but given the habit of the Blonde in a Red Dress to do stuff like chase after a witch’s hoard, I wouldn’t be surprised if this was an actual attack by a literal giant skull monster.
